Patients should pick up any prescriptions prior to their procedure and should arrange for a friend or family member to drive them home after surgery and stay with them for the first day or two. Since traditional facelift procedures are performed under general anesthesia, patients will typically need to refrain from eating or drinking after midnight the night before surgery.
During the Procedure
Depending on the patient’s preferences and on the extent of the surgery, the facelift procedure will begin with either intravenous sedation or general anesthesia.
For patients getting a traditional facelift surgery, the incision will start above and in front of the ear. It moves down around the ear and extends back through the hairline. The incisions will be carefully placed in natural creases near the ears.
After the incisions have been made, the skin is then moved out to the temples, cheeks, and neck so that underlying tissues can be repositioned and tightened before the excess skin is removed.
Fat may or may not be removed, depending on the patient’s natural facial anatomy, and a small cut may be made under the chin to access tissues in the jowls and chin area. Once the tissues have been tightened and contoured, drainage tubes may be placed. Finally, the skin edges will be secured and the dressings applied before the patient is removed to the recovery area. Although results may vary, an experienced surgeon can guarantee positive results almost every time.
After the Procedure: Recovery
After a facelift in Dearborn, patients will usually experience a moderate amount of pain and discomfort for the first 48 hours, but at no point should they be in severe pain. Discomfort should be controllable with oral medications and will typically begin to fade within 48 hours.

Preparation for Rhinoplasty
For best results, the Masri Clinic suggests a cessation of tobacco use several weeks before and after the rhinoplasty in order to minimize the risk of complications. Prior to surgery, patients should stop using certain supplements and medications such as blood thinners as directed by the staff at the clinic. This is usually recommended about two weeks before the procedure.
As it is important for patients to rest after the procedure, it is a good idea to fill any prescriptions and prepare the patient’s home for recovery before the day of the surgery. Finally, patients will need to be driven to and from their surgical appointment at Masri Clinic for safety reasons.
The Rhinoplasty Procedure: Open vs Closed Rhinoplasty
Typically, rhinoplasty is performed under local anesthetic rather than general anesthesia and incisions are often placed inside the nose to hide them. A closed, or endonasal procedure, places all incisions within the nostrils to avoid visible scarring. Bone and cartilage are then shaved and shaped to achieve the desired look. The closed approach offers limited visibility and narrow surgical openings.
The open rhinoplasty approach also uses incisions inside the nostrils, along with another small incision at the base of the nose which allows for improved access and greater accuracy.
However, the open approach may potentially leave a 4-5 millimeter scar at the base of the nose after the patient’s ‘nose job’. This approach is used for correcting significant defects and trauma to the nose while continuing to provide the opportunity for aesthetic work.
OPEN VS. CLOSED
Open Rhinoplasty:
- “External”: This is easier to perform as the doctor has more visibility and access to the nasal structure. Success rate and symmetry is higher as the doctor can observe the position of the nose’s components.
- The incision or cut is done in between the nostrils, known as the columella.
- The incision is kept small to ensure that scarring is at a minimum
- Sutures are done on the outside
Closed Rhinoplasty:
- “Endonasal”: This procedure can be more difficult for less experienced doctors, as the access and visibility is significantly less than what they would have with an open procedure.
- The incision or cut is done inside of the nostrils. This means that there is no visible scar produced by the rhinoplasty, as the cuts are all inside the nostril
- Lesser swelling when complete
- Shorter recovery time due to the type of incision
Options for augmenting the patient’s nose include adding cartilage. Autologous cartilage grafts taken from the nasal septum integrate best but grafts from ear or rib cartilage may also be used.
Silicone as an option includes a higher risk of reacting poorly with existing tissues or of infection. Fillers or fat grafts are also an option to add volume in some areas, but not all of these solutions are permanent. Dr. Masri discusses the surgical plan in detail with patients before performing rhinoplasty.
Recovering and Recovery Time
A nasal splint will shield the patient’s nose from accidental bumps for the first week of healing. Surgical packing inside their nose will remain for several days to support the tissue while it heals in the new position.
Much of the swelling and bruising around the eyes will subside in three or four days during the rhinoplasty recovery. Any residual swelling will gradually disappear in the months following surgery. The nose will retain a small amount of swelling for up to a year. This swelling is minor and generally not noticeable. To help with swelling, it may be beneficial to use skin care products during this period.
The final result of a rhinoplasty is not fully apparent until about a year after surgery.
After a rhinoplasty, patients should be careful to avoid strenuous exercise for three to six weeks because elevated blood pressure associated with exercise will increase their swelling and bruising. They should be able to resume normal social activities within three weeks. Most patients return to work in 7-14 days.

Facial Fat Injection Procedure
Fat transfers are usually performed with local anesthetic and with a sedative that keeps you relaxed. Patients with a strong preference for general anesthesia may be provided that option. Fat is removed from your donor site and is prepared in a manner that keeps as many fat cells alive as possible during the transfer. Under either a general anesthetic or with sedation, fat is removed from the selected area, often the stomach. A mechanical separation process is applied prior to injecting the fat into the desired region. Viability and longevity of the transferred fat are enhanced by using a gentle fat harvesting technique together with a microdroplet injection technique. With this technique, fat is injected evenly and deeply into the tissue layers so that an adequate blood supply supports the injection.

Immediately after the procedure, you will notice swelling around the treatment areas. Expect some of the fat injected to be absorbed by your body. Slightly more fat than necessary may be injected to achieve the final result or subsequent fat injections may be needed in the future. Blepharoplasty, or eyelid surgery, can correct these appearance issues by removing excess skin and fat from the eyelids. As one of the first things that most people look at, your eyes have a big impact on your face. Procedure
Your blepharoplasty will begin with sedation. In most cases, eyelid surgery is performed with a local anesthetic and intravenous sedation, but general anesthesia is available for patients who prefer it.
Once you are sedated, Dr. Masri will begin by making the appropriate incisions on the crease of your upper lid, inside the lower lid, or on the lash line of the lower lid. These will be carefully placed in such a way as to blend with the natural contours of your eyelids to hide any scarring. He will then remove excess skin and fat to reduce sagging, remove crepey skin, reduce bags, and correct other issues.
Once everything has been carefully recontoured, the incisions will be closed, dressings will be placed, and you will be sent off to the recovery room.
Recovery
For the first few days after your surgery, there will be swelling and bruising around your eyes. Many patients experience blurred or double vision due to the swelling, as well as tearing issues like dry eyes or excessive tearing. These problems typically resolve within a week or two. Ice packs and eye drops will both be useful to reduce these side effects. In most cases, pain is quite minimal and should be easily controllable with over-the-counter medications as long as you avoid drugs like aspirin that can increase bleeding.
After surgery, you should take care to keep your head elevated. This means sleeping in a reclined position rather than lying down for the first few days. Strenuous activities should be avoided since they can raise blood pressure and increase bleeding and swelling. In most cases, patients can return to work within two weeks. Your incisions may be visible for a while, but the scars will fade for up to a year.
Possible Risks and Complications
Like all surgeries, eyelid surgery brings a slight chance of bleeding, infection, negative reactions to anesthesia, and hematoma. There are also chances of changes in sensation like numbness or tingling or visible scarring. Both of these problems normally resolve on their own within a year. Visible scarring can be minimized with proper care such as avoiding unprotected sun exposure.
Blepharoplasty also has two more specific risks: Dry eyes and malformations. Dryness of the eyes is very common in the first week or two after surgery due to friction from the swelling, but in a few cases, it persists for longer, leading to potential damage to the corneas. Any lingering dryness should be brought up during your follow-up visits so that it can be corrected. In extreme cases, surgical correction may be necessary.
Malformations of the lower eyelid are another rare but possible complication. This normally consists of the lower lid tightening too much after surgery, which can cause incomplete lid closure. While this sometimes resolves on its own as swelling decreases, lasting malformation will require surgical correction.

The Results to Expect: Since the collagen does not remain permanently at the fold or depression, the patient may choose to repeat the injection every six months or so. With lipoinjection, the body absorbs much of the injected fat, so it may be repeated about three times at three- to six-month intervals. Soft facial implants are non-allergenic synthetic material that is injected through a specific device and typically used in deeper lines.
The fillers are typically injected through a fine needle with minimal discomfort. Local or topical anesthesia can be utilized to minimize the discomfort or to minimize the pain. These injections have to be repeated periodically depending on the filler utilized. Among all these fillers the fat injection appears to be the most natural at this point. Restylane® is a very popular filler that has gained popularity in the past year or two. It is a hyaluronic acid clear gel that is injected through a fine micro-needle.
Typically it is applied after regional or topical anesthesia. Some swelling is experienced for a day or two and normal daily activities are resumed almost immediately after the procedure which takes only a few minutes. This has proven to be very effective especially in lip enhancement lasting almost twice as long as standard collagen implants. This material is not extracted from animals so there is no risk of allergic reaction and no allergy test is required. Hyaluronic acid is a material that is present in the skin and is natural to it and injecting it is natural. This product has been used in Europe and Canada for many years and proved its safety before it was approved by the FDA in the United States. Maintenance therapy typically is required after 6 to 12 months.

Sclerotherapy is for the larger spider veins which are most commonly found on the thigh or lower legs and most frequent in women. This is a hereditary condition and it could be also seen on the face as well.
These are different from varicose veins which are larger and typically are raised above the skin level and are bluish and can be painful.
Both spider veins and the small varicose veins can be eliminated with a simple Detroit office procedure called Sclerotherapy.
Sclerotherapy has been practiced for over 18 years. The procedure consists of inserting a very fine needle with a sclerosing solution into the small vein and typically it makes the vein turn white and the vein will gradually disappear afterwards.
For larger varicose veins, surgery would be needed and Sclerotherapy would not be ideal. Sclerotherapy typically would have to be repeated at least twice with 3 weeks in between.
After the procedure, elastic stockings will have to be worn to reduce bruising and keep the veins closed.
Within 24 hours after the procedure walking is encouraged. However vigorous activities are discouraged. It could take weeks to months before the final results are experienced.

Acne is a very common condition that many of us have experienced at a certain period of our lives, especially in the teenage years. This is caused by the over production of the sebum (oil) in the deep layer of the skin. This can be aggravated by certain high carbohydrate low fiber diets. Certain foods like caffeine and chocolate can increase it. Stress is also an aggravating factor.
Bacteria called P. acnes grows in the plugged follicles causing the acne. If the hair follicle is blocked this leads to a formation of a whitehead, however if it is open this will cause reaction with the oxygen in the atmosphere leading to blackish dirty color what is known as blackhead. There are many approved treatments for acne including topical cleansers, topical antibiotics, oral antibiotics, comedolytics such as Retin-A, Estringents and exfolliants:
Photodynamic Therapy
This includes the painting of a solution known as Levulenic acid (ALA). Then, after an incubation period where the acid remains on the skin, the area is exposed to LED Blue Light. There will be a reaction on the skin that will subside within a period of a week leading to death of most of the bacteria.
This is a very successful treatment for sun-damaged skin including precancerous conditions also for hyper-pigmentations caused by sun damage.
This is a new state of the art treatment that is performed with application of aminolevulinic acid or Levulan® Kerastick to treat skin damage in a gentler way. After applying this material for a certain period of time, specific light is delivered to the skin at a certain wavelength either by using LED Blue Light or Intense Pulsed Light. This will produce redness and an inflammatory reaction that will restore smoother, healthier, and younger looking skin.
This is a very successful treatment for sun-damaged skin including precancerous conditions also for hyper-pigmentations caused by sun damage. These treatments are recommended at three week intervals and the number of treatments necessary will vary and depend on the condition of the skin.
The patient will experience some burning sensation during the treatment and mild swelling and moderate to severe redness. This will look like a severe sunburn and heal typically between five to seven days.
It is important that the patient avoids all sunlight for two days after treatment. There will be significant fading of all sunspots and decrease in the facial redness and the fine lines as new collagen forms to increase the thickness of the skin and improve its condition.
This method has become very popular in treating acne especially the severe sebaceous cystic acne. This treatment has also become popular for treating rosacea. Possible Risks and Side Effects

Blue Light Therapy
Acne occurs when the sebaceous glands that produce the oil result with entrapment of the oil in it. This plugged follicle will allow bacteria to grow creating pimples. If the pore stays open, the top surface of the plug becomes dark causing a blackhead. If it becomes closed then it stays white resulting with a whitehead. This allows the growth of a bacteria called P. acnes resulting with puss leaking out of the follicle causing a pimple. Acne is very common and almost all of us will experience it at a certain time in our life. Three out of four teenagers will experience it to a degree. This could persist throughout life in the people who have more oily skin. This process increases during menstrual periods and can be triggered again during menopause.
Treatment: other treatments can include topical or systemic antibiotics, Retin-AT, exfoliants, and astringents. AccutaneT has also been used successfully however recently being more restricted.
Acne typically will result with lower self-esteem and depression.
Blue light is a new way to treat mild to moderate acne and it is FDA approved and can be covered by health insurance in Detroit, MI. The Fractional Resurfacing Procedure
Fractional laser resurfacing removes layers of skin tissue in a fractionated method in which columns of skin are treated, leaving the skin surrounding each column intact to aid in healing.
During your treatment, a Co2 laser will vaporize the superficial and outermost layer of your skin. Bursts of laser energy are computer controlled in a pre-defined pattern such as a square, circle, rectangle or linear pattern best suited for your skin. The laser creates a controlled “burn” without thermal heat which results in a less damaging procedure. The laser resurfacing process controls the depth of your skin treatment which creates the opportunity to address of the delicate tissues surrounding your eyelids.
Recovery after CO2 Fractional Laser Resurfacing
The skin will continue to improve for a period of six months following the procedure provided proper skincare is used and sun exposure is minimized. Sometimes the results are not significant until the sixth and even the ninth month, depending on the person's age, their aftercare routine and their history of smoking.
After the treatment, most patients describe the post-procedure sensation as being similar to a bad sunburn for about twelve to twenty-four hours. Healing may require seven to fourteen days. Co2 fractional laser resurfacing is performed in our office. Both pre-treatment and post-treatment medications will ensure proper healing and comfort during and after the procedure.
Discomfort associated with the procedure can be expected to vary depending upon the depth of the treatment. For your comfort during the treatment, you may receive a topical anesthetic, local anesthetic, or tumescent anesthetic, possibly combined with sedation. Patients generally rest after the procedure. Your skin will be raw. You will be provided instructions for maximum healing.
After the procedure we like to see our patients each day for three days in order to ensure that healing is progressing. You can usually apply makeup or venture out in public on day seven. However, your skin will be pink and sensitive for one to three months. A skincare regimen and sun avoidance is recommended for best healing and optimal results for a period of six months following treatment. Sub-optimal results or unwanted side effects may occur if you do not follow post-procedure instructions.
Your skin will continue to age from sun exposure, environmental stress or overall health and lifestyle choices such as smoking. To maintain laser results we recommend a healthy skincare routine including cosmeceutical products and facials which are medical grade.

One laser treatment with Co2 fractional laser resurfacing is generally recommended. Occasionally, patients may have an area where aggressive treatment is not suggested. In that event, more than one treatment may be required in order to optimize results. Maintenance treatments may also be necessary to maintain your results, particularly if the sun is not avoided or the patient does not engage in proper skin care used. In some cases, an additional treatment may be required one to two years after the initial treatment in the area around the eyes.
Co2 fractional laser resurfacing can be combined with other treatments but not in the same area(s) on the same day. Wrinkle relaxers can be combined to reduce unwanted wrinkles caused by muscle contraction. Dermal fillers can be used to fill areas where volume has been lost such as the lips, cheeks and lower eyelids. Botulinum toxin two weeks prior to the treatment may enhance the effect on wrinkles caused by movement.
