Facelift Techniques

A common misunderstanding of the facelift procedure is its area of coverage. Most patients generally believe that a facelift is a complete treatment of the face and its three areas – the upper face, midface, and lower face.


However, the typical facelift procedure generally only works on the lower half of the face, or from the tips of the ears down. For work on the upper half of the face, surgeons employ a procedure known as an upper face lift, also known as a brow lift. These two other type of facelift procedures can be performed in the same sitting if required.

QuickLift Mini Facelift

Another variation of the facelift procedure is the limited incision or QuickLift mini lift (one hour face lift), which is faster and less invasive than the full facelift. With the mini face lift, all work is done through small incisions, which means less recovery time and only a small number of short scars. It is also known as the short-incision facelift.


This 1-hr face lift minimizes scarring and reduces sagging skin but will not eliminate loose folds. The results typically last about five years. There are also specific versions like the neck lift suitable for patients whose problems are localized to one small area.

The Facelift Experience: CONSULTATION

A patient’s first step when considering a facelift in Dearborn is a consultation with Dr. Masri at Masri Clinic for Laser and Cosmetic Surgery. During the consultation, patients will talk about their needs and aesthetic goals, and Dr. Masri will go over the types of facelifts available to them.


Their face will be examined, measured and photographed. In addition, there will be a discussion of their aesthetic vision and the likely outcomes of their facelift surgery. Dr. Masri may also share before and after photos of real patients and answer any questions a patient may have. During this meeting, patients will find out if they are a good candidate for facial rejuvenation surgery.


Medical conditions, previous medical treatments or surgeries, and drug allergies will be documented together. There will be a review of the patient’s current medications, herbal supplements, vitamins, alcohol, tobacco, and drug use before their facial surgery is scheduled.

Before the Procedure: Preparing for a Facelift

For best results and to help ensure patient safety, Masri Clinic suggests that patients stop using tobacco products several weeks before and after their facelift procedure in order to minimize the risk of complications.


Patients may need to stop taking some medications and supplements a few weeks before their facelift procedure to prevent excessive bleeding. Masri Clinic will provide patients with all the necessary information at the time of scheduling.



Patients should pick up any prescriptions prior to their procedure and should arrange for a friend or family member to drive them home after surgery and stay with them for the first day or two. Since traditional facelift procedures are performed under general anesthesia, patients will typically need to refrain from eating or drinking after midnight the night before surgery.

During the Procedure

Depending on the patient’s preferences and on the extent of the surgery, the facelift procedure will begin with either intravenous sedation or general anesthesia.


For patients getting a traditional facelift surgery, the incision will start above and in front of the ear. It moves down around the ear and extends back through the hairline. The incisions will be carefully placed in natural creases near the ears.


After the incisions have been made, the skin is then moved out to the temples, cheeks, and neck so that underlying tissues can be repositioned and tightened before the excess skin is removed.


Fat may or may not be removed, depending on the patient’s natural facial anatomy, and a small cut may be made under the chin to access tissues in the jowls and chin area. Once the tissues have been tightened and contoured, drainage tubes may be placed. Finally, the skin edges will be secured and the dressings applied before the patient is removed to the recovery area. Although results may vary, an experienced surgeon can guarantee positive results almost every time.

After the Procedure: Recovery

After a facelift in Dearborn, patients will usually experience a moderate amount of pain and discomfort for the first 48 hours, but at no point should they be in severe pain. Discomfort should be controllable with oral medications and will typically begin to fade within 48 hours.


There will be a considerable amount of bruising and swelling, as well as liquid drainage for a couple of days. For the first 48 hours or so, the patient will need to wear a bulky dressing to prevent blood from collecting under the skin. This dressing is generally replaced with an elastic face garment to support the tissues while they heal for several more days.


For the first few weeks, the patient will need to recline rather than lie down. This helps to hold down the swelling, which will peak around 48 hours post-surgery. It will begin to decline after that, but most patients do not feel comfortable resuming normal social activities for 10-14 days after surgery due to remaining swelling and bruising.


A patient’s activities will be restricted for some time after a facelift. They should not drive or exercise for the first week, but they should try to stay vertical as much as they can to help with the swelling.


Water activities should be limited to brief showers for a full month after surgery, and strenuous exercise should be avoided. The patient will also need to stay out of the sun as much as possible. Strong sunscreen and wide-brimmed hats are recommended to minimize scarring.


In most cases, it will take about six months for everything to heal up fully and up to a year before the scars fade to their final shade.

What are the Possible Side Effects or Risks of Facelift?

Since a facelift is a surgical procedure, it does involve some side effects and risks including:

In fact, having a facelift done younger may even yield some benefits in terms of having better skin elasticity.

  • HOW CAN I AVOID THAT PULLED-TIGHT FACELIFT LOOK?

  • CAN A FACELIFT OR SKIN ONLY FACE LIFT CORRECT ASYMMETRIC FACIAL FEATURES?

    A facelift is not the best procedure to correct asymmetric features because facial asymmetry typically is caused by small differences in the bone structure on each side of the face.   • THERE ARE “NATURAL FACELIFT CREAMS” ADVERTISED EVERYWHERE. DO THEY WORK?

    Unfortunately, no. Some topical creams such as retinoids do have proven anti-aging benefits, and other non-surgical options like Botox® can reduce wrinkles, but if the problem is drooping facial skin, no cream will help.

  • I’VE JUST HAD A FACELIFT AND MY SKIN FEELS LIKE IT HAS BEEN PULLED TOO TIGHT. WILL THIS GET BETTER?

What is Rhinoplasty?

The Rhinoplasty Experience: CONSULTATION

Nose job treatments can be tricky, but with Dr. Masri you are in experienced and trusted hands. During a rhinoplasty consultation Dr. Masri will evaluate the patient’s nose and face and assess their facial balance. If the patient is experiencing breathing problems, the internal nasal structures will be examined as well.


Dr. Masri will ask the patient about their goals for nostril or nasal surgery and discuss realistic changes that could help create the right nose for the patient’s face.


The consultation will also give Dr. Masri a chance to go over the patient’s health history and ensure that they are qualified rhinoplasty candidates. This is an excellent opportunity for patients to ask any questions they may have and get all the information they need to make a decision about whether or not to move forward with rhinoplasty.

Preparation for Rhinoplasty

For best results, the Masri Clinic suggests a cessation of tobacco use several weeks before and after the rhinoplasty in order to minimize the risk of complications. Prior to surgery, patients should stop using certain supplements and medications such as blood thinners as directed by the staff at the clinic. This is usually recommended about two weeks before the procedure.



As it is important for patients to rest after the procedure, it is a good idea to fill any prescriptions and prepare the patient’s home for recovery before the day of the surgery. Finally, patients will need to be driven to and from their surgical appointment at Masri Clinic for safety reasons.

The Rhinoplasty Procedure: Open vs Closed Rhinoplasty

Typically, rhinoplasty is performed under local anesthetic rather than general anesthesia and incisions are often placed inside the nose to hide them. A closed, or endonasal procedure, places all incisions within the nostrils to avoid visible scarring. Bone and cartilage are then shaved and shaped to achieve the desired look. The closed approach offers limited visibility and narrow surgical openings.


The open rhinoplasty approach also uses incisions inside the nostrils, along with another small incision at the base of the nose which allows for improved access and greater accuracy.


However, the open approach may potentially leave a 4-5 millimeter scar at the base of the nose after the patient’s ‘nose job’. This approach is used for correcting significant defects and trauma to the nose while continuing to provide the opportunity for aesthetic work.

OPEN VS. CLOSED

Open Rhinoplasty:

  • “External”: This is easier to perform as the doctor has more visibility and access to the nasal structure. Success rate and symmetry is higher as the doctor can observe the position of the nose’s components.
  • The incision or cut is done in between the nostrils, known as the columella.
  • The incision is kept small to ensure that scarring is at a minimum
  • Sutures are done on the outside

Closed Rhinoplasty:

  • “Endonasal”: This procedure can be more difficult for less experienced doctors, as the access and visibility is significantly less than what they would have with an open procedure.
  • The incision or cut is done inside of the nostrils. This means that there is no visible scar produced by the rhinoplasty, as the cuts are all inside the nostril
  • Lesser swelling when complete
  • Shorter recovery time due to the type of incision

Options for augmenting the patient’s nose include adding cartilage. Autologous cartilage grafts taken from the nasal septum integrate best but grafts from ear or rib cartilage may also be used.


Silicone as an option includes a higher risk of reacting poorly with existing tissues or of infection. Fillers or fat grafts are also an option to add volume in some areas, but not all of these solutions are permanent. Dr. Masri discusses the surgical plan in detail with patients before performing rhinoplasty.

Recovering and Recovery Time

A nasal splint will shield the patient’s nose from accidental bumps for the first week of healing. Surgical packing inside their nose will remain for several days to support the tissue while it heals in the new position.


Much of the swelling and bruising around the eyes will subside in three or four days during the rhinoplasty recovery. Any residual swelling will gradually disappear in the months following surgery. The nose will retain a small amount of swelling for up to a year. This swelling is minor and generally not noticeable. To help with swelling, it may be beneficial to use skin care products during this period.

The final result of a rhinoplasty is not fully apparent until about a year after surgery.

Preparation for Facial Fat Injections

In preparation for your facial fat injection, smoking cessation is recommended several weeks prior to your injections in order to maximize healing and to avoid scarring. Avoid aspirin and other drugs that might cause excess bleeding.

Facial Fat Injection Procedure

Fat transfers are usually performed with local anesthetic and with a sedative that keeps you relaxed. Patients with a strong preference for general anesthesia may be provided that option. Fat is removed from your donor site and is prepared in a manner that keeps as many fat cells alive as possible during the transfer. The fat is then carefully injected into your recipient sites with aesthetic balance and symmetry guiding their location. The procedure is often completed within an hour but can take longer based upon the amount of fat transferred.


Under either a general anesthetic or with sedation, fat is removed from the selected area, often the stomach. A mechanical separation process is applied prior to injecting the fat into the desired region. Viability and longevity of the transferred fat are enhanced by using a gentle fat harvesting technique together with a microdroplet injection technique. With this technique, fat is injected evenly and deeply into the tissue layers so that an adequate blood supply supports the injection.


Many patients opt for stem cells to enrich the fat transfer which involves mixing the fat with a heavy concentration of your body’s own stem cells, extracted from your fat cells. Post-Procedure Recovery Expectations for Facial Fat Transfer

Full results are evident within one week and can last one to several years, even longer in many cases due to our special fat harvesting and injection techniques.


Immediately after the procedure, you will notice swelling around the treatment areas. Expect some of the fat injected to be absorbed by your body. Slightly more fat than necessary may be injected to achieve the final result or subsequent fat injections may be needed in the future. Typically, 30 percent to 50 percent of the fat that is initially injected will reabsorb over time; however, because of the additive nature of the fat transfer, subsequent fat injections tend to reabsorb at a lower rate yielding longer lasting results.

    Another risk is an insufficient transfer of fat, which may contribute to sagging, though the remaining skin will likely show improvement attributable to the tightening effect of stem cells and collagen. If the new contour is not acceptable, another injection may be necessary to correct the re-absorption.


    Over-correction may occur if too much fat is transferred.  In a small over-correction case, a fat-dissolving solution may be the first option.  If that is not effective, excess fat will require surgical removal via excision or liposuction.


    Facial asymmetry is a risk which is easiest to correct by injecting a bit more fat on the smaller side of your face.

Blepharoplasty

Blepharoplasty, or eyelid surgery, can correct these appearance issues by removing excess skin and fat from the eyelids. This decreases sagging and under-eye bags, which can make your entire face look alert, awake, and years younger. Eyelid surgery can also correct some texture issues like crepey skin or grooves under the eyes.


As one of the first things that most people look at, your eyes have a big impact on your face. Blepharoplasty is available for both the upper and lower lids, depending on your specific combination of problems. Sagging of the upper lid is always corrected the same way with the incision placed at the crease of the eyelid to allow removal of skin and fat while camouflaging the scar.


For under-eye issues, Dr. Masri will walk you through the incision options available. The traditional blepharoplasty uses an incision along the lash line to allow full removal of excess skin as well as fat. If your only issue is excess fat causing the appearance of bags, there is also a transconjunctival incision, which is placed just inside your eyelid to allow removal of fat but not skin. This incision leaves no visible scar.


Dr. Masri will also discuss other procedures that you may wish to have at the same time as your eyelid surgery. Many patients have the procedure in conjunction with a face lift, a brow lift, or fat transfers or fillers to plump the area and further reduce signs of aging.



Once you've settled on the details of your procedure, you'll also go over your medical history as well as any precautions that you may wish to take to reduce the normal risks of surgery.

Procedure

Your blepharoplasty will begin with sedation. In most cases, eyelid surgery is performed with a local anesthetic and intravenous sedation, but general anesthesia is available for patients who prefer it.


Once you are sedated, Dr. Masri will begin by making the appropriate incisions on the crease of your upper lid, inside the lower lid, or on the lash line of the lower lid. These will be carefully placed in such a way as to blend with the natural contours of your eyelids to hide any scarring. He will then remove excess skin and fat to reduce sagging, remove crepey skin, reduce bags, and correct other issues.



Once everything has been carefully recontoured, the incisions will be closed, dressings will be placed, and you will be sent off to the recovery room.

Recovery

For the first few days after your surgery, there will be swelling and bruising around your eyes. Many patients experience blurred or double vision due to the swelling, as well as tearing issues like dry eyes or excessive tearing. These problems typically resolve within a week or two. Ice packs and eye drops will both be useful to reduce these side effects. In most cases, pain is quite minimal and should be easily controllable with over-the-counter medications as long as you avoid drugs like aspirin that can increase bleeding.



After surgery, you should take care to keep your head elevated. This means sleeping in a reclined position rather than lying down for the first few days. Strenuous activities should be avoided since they can raise blood pressure and increase bleeding and swelling. In most cases, patients can return to work within two weeks. Your incisions may be visible for a while, but the scars will fade for up to a year.

Possible Risks and Complications

Like all surgeries, eyelid surgery brings a slight chance of bleeding, infection, negative reactions to anesthesia, and hematoma. There are also chances of changes in sensation like numbness or tingling or visible scarring. Both of these problems normally resolve on their own within a year. Visible scarring can be minimized with proper care such as avoiding unprotected sun exposure.


Blepharoplasty also has two more specific risks: Dry eyes and malformations. Dryness of the eyes is very common in the first week or two after surgery due to friction from the swelling, but in a few cases, it persists for longer, leading to potential damage to the corneas. Any lingering dryness should be brought up during your follow-up visits so that it can be corrected. In extreme cases, surgical correction may be necessary.



Malformations of the lower eyelid are another rare but possible complication. This normally consists of the lower lid tightening too much after surgery, which can cause incomplete lid closure. While this sometimes resolves on its own as swelling decreases, lasting malformation will require surgical correction.

Acne scar: The most common type of facial scarring comes from acne infections. Whenever the skin has been broken, whether by an injury, infections such as acne, or even surgery, there always will be a scar.

Common Solutions

Since scar revision procedures may affect patients differently; each scar and each patient require careful study by the facial plastic surgeon before he selects a procedure to improve the scar. The most frequently used procedures are these:


Excision: Although scars cannot be totally eliminated, scar tissue can be removed and wound edges carefully sutured to create an even, narrow scar. Very wide scars or scars in areas of severe tension sometimes may be narrowed in stages, excising only a portion of the scar at a time so that the surrounding skin can accommodate the change.


The term "excision" also includes surgical techniques whereby the facial plastic surgeon repositions or irregularizes the scar so that its appearance may be more easily camouflaged. The surgeon will try to reposition the scar if he can bring it to fall within natural skin lines or creases. If that is not possible, he can irregularize it by breaking up the straight, unnatural line of the scar into small geometric patterns that blend better with the skin texture.


Collagen injections: Scars that lie in appropriate directions and are adequately narrow, but are depressed, can be filled with collagen. Collagen, a natural bovine product, has the advantage of blending well with the deep skin's own tissue makeup. Its disadvantage is that it is not permanent and may need periodic augmentation.


Steroid injection: Some scars - particularly the hypertrophic and keloid scars - are improved by the injection of steroid medication. This medication does not narrow the scar but often will flatten and soften it, achieving a degree of blend with surrounding skin.


Dermabrasion: Some scars may be smoothed or "sanded" to blend with surrounding skin through dermabrasion. This sanding or abrasive technique usually is done with a rotary abrasive instrument. Often, dermabrasion is combined with other scar revision techniques to gain the best final result.

Once the surgery is agreed upon, pre-operative photographs are taken to help the surgeon plan the operation. These photographs usually are compared with similar ones taken sometime after surgery and serve as a permanent before-and-after record of the results.

The Immediate After-Effects

Most patients experience minimal pain or discomfort following scar revision. Neither is it painful when sutures are removed, usually within a few days of surgery and, typically, right in the surgeon's office.

The Results To Expect

Again, scars cannot be removed, only improved, and these improvements can take months. The scar revision patient must be willing to accept this long-term commitment. Once scar treatment has begun, it will require multiple steps, including dermabrasion and other touch-up procedures, plus prolonged use of special facial tape that takes the tension off of the wound edge to prevent scar widening. What is the MINT Lift?

Collagen and elastin, essential components of the skin, are plentiful in youth, but begin to decline with age. Preparing for a MINT Lift

Patients may need to make preparations in the weeks leading up to the procedure. MINT Lift at Masri Clinic

Collagen and Lipoinjection

Common Problems: Deep folds and depressions on the face can result from scars, wrinkles, or areas of atrophy. These can warrant special attention from a facial plastic surgeon.


Common Solutions: Collagen injection and lipoinjection create a smooth, friendly, and youthful appearance by raising deep folds and depressions in your skin. The most frequent sites for injections are along the creases that run from the nose to the corners of the mouth, around the lips, and along the frown lines above the nose. Collagen injections: Collagen, a protein extracted from beef hide, has the advantage of blending well with the deep skin's own tissue makeup. Folds and depressions are raised by injecting collagen into the middle and deep layers of the skin at several points along the fold or depression, using a fine needle. The collagen is massaged into position, leaving the area of the fold slightly puffy. Lipoinjection: Fat taken from another part of the body (such as the abdomen, hips, or thighs) is injected just underneath the skin, along the path of the fold or depression. A bandage may be placed over the site of the injection to keep the fat stabilized while the face is healing.


The Immediate After-Effects: Most patients experience minimal pain or discomfort following injections. Most patients put a little makeup on and go back to work the same day. Swelling and puffiness at the site of the injection usually subside within a few days.


The Results to Expect: Since the collagen does not remain permanently at the fold or depression, the patient may choose to repeat the injection every six months or so. With lipoinjection, the body absorbs much of the injected fat, so it may be repeated about three times at three- to six-month intervals. Gradually, enough fat may remain at the site to raise the fold for an extended period.

Facial Fillers

As we age, we lose the supporting layers of our skin which are namely the collagen and elastin fibers resulting with facial wrinkles. Injectable fillers have been utilized for the last thirty years and have become numerous and more effective in the last few years. These can be short or long-lasting depending on the desired results. Collagen injections have been used for many years and have been proved to be safe and effective. Millions of peoples have used these fillers in the past few years. Wrinkle fillers have varied from utilizing autologous fat (fat from your own body) to utilizing bovine collagen pastes like Zyderm® & Zyplast®, Artecoll®, Fascian®, Cymetra®, and AlloDerm®, and human collagen fillers such as CosmoDermT and CosmoPlastT. Soft facial implants are non-allergenic synthetic material that is injected through a specific device and typically used in deeper lines.


The fillers are typically injected through a fine needle with minimal discomfort. Local or topical anesthesia can be utilized to minimize the discomfort or to minimize the pain. These injections have to be repeated periodically depending on the filler utilized. Among all these fillers the fat injection appears to be the most natural at this point. Restylane® is a very popular filler that has gained popularity in the past year or two. It is a hyaluronic acid clear gel that is injected through a fine micro-needle.


Typically it is applied after regional or topical anesthesia. Some swelling is experienced for a day or two and normal daily activities are resumed almost immediately after the procedure which takes only a few minutes. This has proven to be very effective especially in lip enhancement lasting almost twice as long as standard collagen implants. This material is not extracted from animals so there is no risk of allergic reaction and no allergy test is required. Hyaluronic acid is a material that is present in the skin and is natural to it and injecting it is natural. This product has been used in Europe and Canada for many years and proved its safety before it was approved by the FDA in the United States. Maintenance therapy typically is required after 6 to 12 months.

Mesotherapy

Although dieting and exercise may significantly help reduce fat, they fail to resolve fat problems in stubborn areas.


The art of dissolving fat: As we gain weight, fat is deposited in many unwanted areas in the face and body. Although dieting and exercise may significantly help reduce fat, they fail to resolve fat problems in stubborn areas including the jawline, neck, chin, love handles and hips. Mesotherapy is an FDA approved method that has been used in France since 1952. Vitamins, minerals and nutrients are injected into the layer of fat just under the skin (the mesoderm). The injection dissolves this layer of fat and absorbes it into the blood stream. Mesotherapy can also be used to dissolve the fibrous material that causes cellulite, leaving smoother skin.


Typically 5 to 7 treatments are necessary to achieve proper results. Maintaining a healthy diet and exercise enhances the results. After the procedure, the results are gradual and can be measured in inches or pounds. Results can average as much as 1 pound per week after treatment. Best of all, there is no downtime! Sclerotherapy

Sclerotherapy is for the larger spider veins which are most commonly found on the thigh or lower legs and most frequent in women. This is a hereditary condition and it could be also seen on the face as well.


These are different from varicose veins which are larger and typically are raised above the skin level and are bluish and can be painful.

Both spider veins and the small varicose veins can be eliminated with a simple Detroit office procedure called Sclerotherapy.


Sclerotherapy has been practiced for over 18 years. The procedure consists of inserting a very fine needle with a sclerosing solution into the small vein and typically it makes the vein turn white and the vein will gradually disappear afterwards.


For larger varicose veins, surgery would be needed and Sclerotherapy would not be ideal. Sclerotherapy typically would have to be repeated at least twice with 3 weeks in between.


After the procedure, elastic stockings will have to be worn to reduce bruising and keep the veins closed.


Within 24 hours after the procedure walking is encouraged. However vigorous activities are discouraged. It could take weeks to months before the final results are experienced.


The procedure typically carries very little discomfort and there could be some mild burning sensations during the procedure, but it stops within a few seconds.

Botox Therapy

Facial wrinkles occur due to an intimate combination of aged, sun-damaged skin and the chronic action of the underlying muscles. BOTOX THERAPY FAQ

  • HOW DOES BOTULINUM TOXIN WORK?

    Botulinum Toxin works by selectively and temporarily paralyzing the muscles into which it is injected. The rationale for its use is that one can inject a chemical which selectively inhibits the action of muscles in certain hyperdynamic areas (like the forehead), then this in turn will eliminate the wrinkles which are caused by that muscle action. In the case of excessive sweating, Botulinum Toxin prevents signaling to sweat glands and normalizes sweat production.

  • HOW LONG DO THE BOTULINUM TOXIN INJECTIONS LAST, AND ARE ADDITIONAL TREATMENTS REQUIRED?

    The effect of one Botulinum Toxin treatment usually lasts 3 to 4 months, after which re-treatment is required. After three to four simultaneous treatments, however, the Botulinum Toxin effects become more long lasting, as the muscles that cause the wrinkles begin to lose their strength and develop atrophy. Minimizing the use of treated muscles by controlling animation will help Botulinum Toxin last longer. The best time to re-treat is as soon as the muscle is starting to move. In the case of excessive sweating, re-treatments are usually needed after 6-9 months.

  • WHAT ARE THE POSSIBLE SIDE EFFECTS?

    Temporary bruising is possible and is minimized by avoiding aspirin-like products and Vitamin E for 1 week before injections and alcohol for 3 days before injections. Mild headaches can occur and usually resolve with Tylenol. A slight change in the shape of the eyebrow is possible. Drooping of the eyelid is rare and usually resolves within a few days.

  • ARE THERE ANY POST-INJECTION PRECAUTIONS?

    You should keep your head elevated for 3 hours if you have the frown lines treated (so pick up your bag while bending at the knees, after treatment). It is important to utilize the treated area for the first 3 hours to help the drug diffuse well. Mild exercise is allowed 4 hours after the treatment.

  • HOW QUICKLY DOES IT ACT?

    To put it plainly, Botox resistance (immunity) is not common, but does occur in some patients. According to those who do recognize the phenomenon of Botox resistance, the numbers are low -– about 1% to 3% of patients who are injected will develop toxin-blocking antibodies. Still, with millions of Botox injections being given every year, this number is not insignificant. To minimize the risk of becoming resistant, patients should be given the lowest possible effective dose.

Acne Treatments

Acne is a very common condition that many of us have experienced at a certain period of our lives, especially in the teenage years. This is caused by the over production of the sebum (oil) in the deep layer of the skin. This can be aggravated by certain high carbohydrate low fiber diets. Certain foods like caffeine and chocolate can increase it. Stress is also an aggravating factor.


Bacteria called P. acnes grows in the plugged follicles causing the acne. If the hair follicle is blocked this leads to a formation of a whitehead, however if it is open this will cause reaction with the oxygen in the atmosphere leading to blackish dirty color what is known as blackhead. There are many approved treatments for acne including topical cleansers, topical antibiotics, oral antibiotics, comedolytics such as Retin-A, Estringents and exfolliants:

Photodynamic Therapy

This includes the painting of a solution known as Levulenic acid (ALA). Then, after an incubation period where the acid remains on the skin, the area is exposed to LED Blue Light. There will be a reaction on the skin that will subside within a period of a week leading to death of most of the bacteria.


This is a very successful treatment for sun-damaged skin including precancerous conditions also for hyper-pigmentations caused by sun damage.


This is a new state of the art treatment that is performed with application of aminolevulinic acid or Levulan® Kerastick to treat skin damage in a gentler way. After applying this material for a certain period of time, specific light is delivered to the skin at a certain wavelength either by using LED Blue Light or Intense Pulsed Light. This will produce redness and an inflammatory reaction that will restore smoother, healthier, and younger looking skin.


This is a very successful treatment for sun-damaged skin including precancerous conditions also for hyper-pigmentations caused by sun damage. These treatments are recommended at three week intervals and the number of treatments necessary will vary and depend on the condition of the skin.

The patient will experience some burning sensation during the treatment and mild swelling and moderate to severe redness. This will look like a severe sunburn and heal typically between five to seven days.


It is important that the patient avoids all sunlight for two days after treatment. There will be significant fading of all sunspots and decrease in the facial redness and the fine lines as new collagen forms to increase the thickness of the skin and improve its condition.


This method has become very popular in treating acne especially the severe sebaceous cystic acne. This treatment has also become popular for treating rosacea. There has been noticeable reduction in the size of the pores of the skin and more even skin tone and significant reduction of the depth of the wrinkles.


Side effects: there has not been significant adverse side-effects seen with the photo-dynamic therapy or PDT with ALA.

  • WHAT IS THE DIFFERENCE BETWEEN DERMABRASION AND MICRODERMABRASION?

    There is almost no comparison between these two procedures. Microdermabrasion is more of a spa treatment to offer a very mild improvement in skin texture. Dermabrasion generally refers to a surgeon performed procedure that is excellent for acne scars or deep wrinkles. However, the recovery time is obviously greater for dermabrasion. Since laser resurfacing came about dermabrasion has become old school. Laser resurfacing has less down time and more results. Laser resurfacing can be repeated as much as the patient likes (as long as there is enough healing time between sessions) whereas Dermabrasion thins the skin so the procedure can only be repeated a few times.

Chemical Peels

A chemical peel uses a solution applied to the skin to remove dead skin cells and stimulate the production of new skin cells. Chemical peels have been used safely for over 100 years. There are numerous medications available for improving acne, skin tone and the texture. A lot of these medications can be done without losing any time off work. They produce minimal redness however they improve the quality of the skin over a span of a few days. Among the most popular chemical peels are:


  1. Glycolic Peel: Glycolic acid is naturally found in grapefruit. This material used in different concentrations can be used to improve acne and fine lines. The procedure is inexpensive and can be repeated on weekly basis for one skin cycle (6 weeks) or more. Lighter concentrations of Glycolic peels can be used by the patient at home on a weekly basis. Also worth noting, you can have this procedure done during pregnancy.
  2. Jessner Peel: This is a solution that has been used for years to revitalize the skin. It results in minimal redness for the first 2 or 3 days and produces excellent results for skin rejuvenation.
  3. Lactic & Carbolic Peels: These peels are considered to be light to medium depth. They help improve fine to medium depth wrinkles, and do not necessitate any time off from work.

Laser Hair Removal and You: Is Laser Hair Removal Right for You?

Traditionally, laser skin hair removal was more difficult to perform on patients who were not the ideal candidate: fair skin with dark hair. This is due to the nature of laser hair removal: to ensure that the heat of the laser is absorbed by the hair instead of the skin, there must be a stark contrast in hair pigmentation and skin type. If the unwanted hair and the skin are similar in color, it is difficult for the laser to target the hair, meaning there is a possibility of skin damage from the laser heat being absorbed by the skin.

During – The Procedure

Possible Risks and Side Effects

  • Temporary Minor Side Effects: You might experience temporary minor side effects that will fade away in a day or two after the procedure. These include slight swelling, redness, and irritation.

    CO2 ultra-pulse, with this procedure, carbon dioxide or CO2 laser is used on a computerized scanning head to achieve tightening of the skin and eliminating deep creases, frown lines and other facial wrinkles. It is a great treatment for deep acne scars. This will result with growth of the more youthful deeper skin and shrinkage of the collagen fibers which are the building blocks of the skin resulting with a younger and smoother skin.


    The procedure is performed under general anesthesia or under sedation, however for smaller areas like around the mouth or the eyelids, only local anesthesia is necessary. This type of laser provides depth control making the procedure much safer and requires shorter recovery.

    Sclerotherapy along with Nd:YAG treatments are used to treat veins of all different sizes. These treatments get rid of veins that are present, but cannot prevent them in the future.

Blue Light Therapy

Acne occurs when the sebaceous glands that produce the oil result with entrapment of the oil in it. This plugged follicle will allow bacteria to grow creating pimples. If the pore stays open, the top surface of the plug becomes dark causing a blackhead. If it becomes closed then it stays white resulting with a whitehead. This allows the growth of a bacteria called P. acnes resulting with puss leaking out of the follicle causing a pimple. Acne is very common and almost all of us will experience it at a certain time in our life. Three out of four teenagers will experience it to a degree. This could persist throughout life in the people who have more oily skin. This process increases during menstrual periods and can be triggered again during menopause.


Treatment: other treatments can include topical or systemic antibiotics, Retin-AT, exfoliants, and astringents. AccutaneT has also been used successfully however recently being more restricted.


Acne typically will result with lower self-esteem and depression.


Blue light is a new way to treat mild to moderate acne and it is FDA approved and can be covered by health insurance in Detroit, MI. Blue light is an LED light that produces a single wavelength which results in the destruction of the bacteria P. acnes inside the pimples causing resolution of the active and sebaceous cystic acne. Blue light alone can be effective however the results will be greatly enhanced by applying aminolevulunic acid or Levulan® Kerastick®.


If Blue Light is utilized without ALA typically will have to be repeated on twice a week basis for a few weeks. If however utilized with ALA it is recommended to be repeated every two to three weeks and the number of treatments depends on the severity of the condition. Without levulunic acid usually there is no significant redness noticed. However with ALA there will be severe redness that will resolve within a few days. This treatment has revolutionized the treatment for acne and it is considered now the gold standard for acne treatments.

co2 laser: Carbon Dioxide Fractional Laser Resurfacing

Carbon Dioxide (Co2) Fractional Laser Resurfacing removes layers of skin tissue in a fractionated method in order to rejuvenate the skin. Co2 fractional lasers have the power to deliver very short pulsed light energy, which is called ultra pulsed, and works with continuous light beams. This type of laser removes thin layers of skin with precision and minimal damage to your surrounding tissue.


During the process, columns of damaged skin are treated, leaving the skin surrounding each column intact to aid in healing. Fractional laser resurfacing helps reduce age-spots, which are also called sun freckles, liver spots and lentigines. Fractional laser resurfacing is an excellent treatment for advanced skin damage and wrinkling addressing uneven coloration, wrinkles, scarring, fine lines, skin laxity, textural irregularities, dull tone, and a thinned collagen layer of the face, neck, and chest.

The Fractional Resurfacing Procedure

Fractional laser resurfacing removes layers of skin tissue in a fractionated method in which columns of skin are treated, leaving the skin surrounding each column intact to aid in healing.



During your treatment, a Co2 laser will vaporize the superficial and outermost layer of your skin. Bursts of laser energy are computer controlled in a pre-defined pattern such as a square, circle, rectangle or linear pattern best suited for your skin. The laser creates a controlled “burn” without thermal heat which results in a less damaging procedure. The laser resurfacing process controls the depth of your skin treatment which creates the opportunity to address of the delicate tissues surrounding your eyelids.

Recovery after CO2 Fractional Laser Resurfacing

The skin will continue to improve for a period of six months following the procedure provided proper skincare is used and sun exposure is minimized. Sometimes the results are not significant until the sixth and even the ninth month, depending on the person's age, their aftercare routine and their history of smoking.


After the treatment, most patients describe the post-procedure sensation as being similar to a bad sunburn for about twelve to twenty-four hours. Healing may require seven to fourteen days. Co2 fractional laser resurfacing is performed in our office. Both pre-treatment and post-treatment medications will ensure proper healing and comfort during and after the procedure.


Discomfort associated with the procedure can be expected to vary depending upon the depth of the treatment. For your comfort during the treatment, you may receive a topical anesthetic, local anesthetic, or tumescent anesthetic, possibly combined with sedation. Patients generally rest after the procedure. Your skin will be raw. You will be provided instructions for maximum healing.


After the procedure we like to see our patients each day for three days in order to ensure that healing is progressing. You can usually apply makeup or venture out in public on day seven. However, your skin will be pink and sensitive for one to three months. A skincare regimen and sun avoidance is recommended for best healing and optimal results for a period of six months following treatment. Sub-optimal results or unwanted side effects may occur if you do not follow post-procedure instructions.



Your skin will continue to age from sun exposure, environmental stress or overall health and lifestyle choices such as smoking. To maintain laser results we recommend a healthy skincare routine including cosmeceutical products and facials which are medical grade.

Consultation with Masri Clinic for Fractional Resurfacing

One laser treatment with Co2 fractional laser resurfacing is generally recommended. Occasionally, patients may have an area where aggressive treatment is not suggested. In that event, more than one treatment may be required in order to optimize results. Maintenance treatments may also be necessary to maintain your results, particularly if the sun is not avoided or the patient does not engage in proper skin care used. In some cases, an additional treatment may be required one to two years after the initial treatment in the area around the eyes.


Co2 fractional laser resurfacing can be combined with other treatments but not in the same area(s) on the same day. Wrinkle relaxers can be combined to reduce unwanted wrinkles caused by muscle contraction. Dermal fillers can be used to fill areas where volume has been lost such as the lips, cheeks and lower eyelids. Botulinum toxin two weeks prior to the treatment may enhance the effect on wrinkles caused by movement.

  • WHO ARE THE BEST CANDIDATES FOR CO2 FRACTIONAL LASER RESURFACING?

    Anyone who has sun damaged, aging skin may be a candidate for Co2 fractional laser resurfacing.  This includes wrinkles, sagging, irregularly pigmented skin, irregular skin texture or other skin conditions that could be expected to be improved with this treatment.  It is important to understand the risks, costs and time necessary to achieve the full benefit of the procedure and have realistic expectations of the ultimate outcome.

  • WHAT ARE THE RISKS AND SIDE EFFECTS OF CO2 FRACTIONAL LASER RESURFACING?

    The Co2 Fractional Laser Resurfacing includes significant downtime.  Depending upon the area and the aggressiveness of the treatment, it will take 7-14 days for your skin to heal.  This downtime will include significant redness and crusting, including possible minor bleeding and oozing depending on treatment depth.  Dryness, peeling and flaking occur and it is important to keep the skin moist after treatment.


    Social and work activities should be curtailed during this time, although you will be capable of working from home after the first day or two.  There can be pain with this procedure but full facial anesthesia minimizes the pain during the procedure and the hours following surgery.  Other potential side effects may include but are not limited to skin wound, infection, scarring, hypo-pigmentation and hyper-pigmentation.

Laser Skin Treatments

With a laser, your doctor can now remove or lighten most freckles, spots, spider veins, and red birthmarks. A laser can also remove tattoos, warts, and some scars and moles. Blemishes in the top layers of skin, such as freckles and spider veins, can often be removed in one treatment. Other blemishes may require two or more treatments. Your Experience

Laser treatment is quick. It is done in your doctor's office or at a surgery center in Detroit. You can most likely go home right afterward.
